    I find it disheartening that so little emphasis be on the nurse not having to handle clarification of the Physician's or P.A 's orders. That, except for the most extreme circumstances everyone from the ordering physician to the nurse administering the medication be exacting during the process; that each phase align to each persons level of expertise, and that rarely if ever should any expectation be that someone else remedy any deviation from their own lapse in performing to the standard of care

    Written Up

    The order was unclear at the time it was written and should have been flagged for clarification first by the nurse taking the order and then by the Pharmacy. In any case the person administering the medication is the last defense and in this instance due to the hour-the dose for sleep, given and the q 8hr.prn dose requested marked as 'not given' and the reason; that way, the patient may have gotten her needed sleep and the creative nurse would not be losing sleep over an unnecessary error.

    Anyone failing to see the charge of RECKLESS Homicide as an apt description of this case is in my non-expert opinion, GUILTY of GROSS NEGLIGENCE, ignorance of the fact that Nurse RV's substandard care was completely and directly responsible for the killing of another; As if the Pyxis suddenly transformed into a gun cabinet and by her flagrant disregard of the most basic measures linked to med error prevention, (identify the correct patient, the correct medication, the correct time, the correct dosage, the correct route) a life may not have been so senselessly taken. Instead, a relatively new nurse loaded her weapon, aimed and fired it, into a hapless victim. The shame of it is sickening, the repercussions to follow, mind-numbing. Wuzzie,succinct as ever
