    EVAL form

Hi everyone, I am a RN who just passed my boards but has been working as an LVN for 15 years the last 6 in hospice. I was a team LVN for 4.5 years and then moved into the intake department. Now that I am an RN, I will be doing evals/admissions. I am looking for help or guidance with some of your favorite resources for evals-especially any forms or brains you like to use in the field when doing evals. My agency has asked me to take on the task of looking at our eval process and making it better. As many of you know hospice is a calling, and after watching high turn over especially in our intake and admission team, I am hoping to help streamline and turn our department into the strongest in the agency. So please share any advice or forms you all find useful. Thanks so much!!

    East Bay

Hello fellow nurses. I currently live in the Central Valley and work as a hospice nurse. I plan on moving back to the east bay next summer. RN wages here range greatly from high 20's to high 40's. Any idea what Rn salaries are in the Bay Area. Specifically hospice in walnut Creek or Brentwood area. Thanks!
