I've seen that phenomenon a few ties before. The smaller twin had much higher endogenous catecholamine levels in utero and therefore was much more ready to be premature than the bigger twin. And Twin B is usually sicker than Twin A in most cases, even when they're older. We see kiddies who are Twin Bs come in deathly ill from RSV or adenovirus who go on to have chronic lung disease, and Twin A might have a bit of a runny nose...
