Pre printed Dr. order form yucks

Published

We have pre-printed doctor orders for s/p colonoscopies or EGD's, this is in addition to a discharge sheet the doctor writes his finding on that we send the patient home with.

I never even read the pre-printed orders, just try to remember to sign them off.

The other day I read it. Thanks goodness I did.....it says "notify physician if Aldrete score is zero".

Gosh I am so glad I read that....I would never have known what to do if the patient was not responding to verbal commands, apenic, not moving their extremities, etc. I guess if the Adrete score was one we'd be okay?

NEVER, EVER... sign a form without reading it first or already know what's in it !!! They make you sign them because they are important and legally binding. By signing you agree to follow what they say.

If the patient suffers an injury, an adverse reaction, or their health deteriorates because you did not do something you were supposed to do... What do you think could happen to your patient? :nailbiting: Who do you think will be held liable when all is set and done? :nailbiting: Whose license is at risk of disciplinary action? :nailbiting: What would you say when it's time for a deposition in a sentinel event? :nailbiting:

You have to think of it in those terms. Patients trust that the entire healthcare team will guide them through a safe recovery. It is up-to us to ensure protocols are being followed. There will always be unfortunate cases in your career, but if you can proof you did everything you could within your scope of practice and followed protocol... you not only gave the best chances to your patient, but you will also be legally protected.

I am glad you posted this experience so others can learn from it too.

Never heard of Alderte score so searched for info on it: What is an Aldrete score?

Glad to see there is an objective scoring system for when one can be safely discharged from a unit post anesthesia.
