post restraint nursing assessment for school

I am a school nurse in the state of PA and have recently been asked to do nursing assessments on all children who have been restrained for behavior issues.

I would like to develop a check off sheet but was hoping there was one out there already in existence.

Just curious what type of school this is that is allowed to restrain kids.

Public school , a few staff members trained in passive restraint for safety purposes.
