Pls Help Regarding pearsonvue nclex registration

please help.... Im wishing to take the nclex exam and this is my 2nd time around. Im done applying for a re-examination from the BON state i applied for, And my re-exam has been granted. While Registering for an ATT at PERSONVUE the second page of the registration confuses me and and wouldnt go to the next page due to lack of information. can anyone here help me with this? "-OR- Program Code (e.g. xx-xxxx)" to see a better picture of it, pls click this site.

you can refer to the application handbook send to you by your respective bon regarding the program code. for the philippines its international education code is 99-425. good luck on your upcoming exam!
