I messed up for the state clinical exam and I am still confused on a few things! The questions I have are in bold, so please read through and help me out.
Giving a Bedpan
What you need:
patient, bedpan, chuck (disposable bed liner), gloves, trash can, soiled linen hamper, washcloth (moistened for patient to clean self), toilet paper
Skill: Giving Bedpan:
Knock before entering the room
Greet the resident: "Good Morning, Mr. _____, my name is, Cathy, I am going to be your Nurse Aide today and I am here to assist you with the bedpan.
"Okay Mr. _____, let me check your id band?"
"Now I am going to wash my hands."
"Now I am going to pull the curtain around so you have some privacy."
[Provide for privacy: close curtains, drapes and doors. Keep resident covered. Expose only area of resident's body necessary to do the procedure]
Inform the resident that you are going to collect your equipment
[get equipment: I need a bedpan, chuck (incontinent pad), used as the cover for the bedpan at the end, gloves, trash can, gloves, soiled linen hamper, moistened washcloth, toilet paper]
Make sure the wheels on the bed are locked
Raise side rails before raising bed to a comfortable working height usually waist high for body mechanics.
Lower the bed rail near you if up
Put your gloves on
Lower the bed flat to supine but raise the head of the bed slightly
Ask the patient to bend their knees and lift up their buttocks
Place the chuck under the patient
Place the bedpan
Say "Okay, now I am just going to ask you to spread your legs a little so it does not get all over the bed."
Cover the resident
Give the resident the toilet tissue and call bell and say "That's for you."
Say "Now I need to raise the head of your bed so that you are sitting up comfortably."
Raise the head of the bed to Fowler's position 45 degrees
Then lower the bed to its lowest position
Say:" I am going to give you privacy now, but ring the call bell when you are done and I will return to assist you," then draw the curtain and walk away.
Announce that you are going to wash your hands.
I go into the bathroom and wash my hands again and put on fresh gloves. Then while I am in there I moisten a washcloth with water to give to the patient.
Give the patient the moistened washcloth.
"Okay now I am just going to raise your bed."
Lower the head of the bed flat to supine position
Ask the patient to lift up their buttocks, so you can remove the bedpan
Lift the bedpan out and put the chuck cover over it (the dirty chuck that was under the patient?)
Take one glove off after you have removed the bedpan (while still holding the bedpan?)
Use the clean hand to fix the bed
Make sure you cover the patient with their blanket.
Use the clean hand to make sure the bed is in the lowest position
Put the bedpan at the end of the bed. (Because during the test there is nothing in it) (what?)
Ask the patient if he is comfortable. Make sure the patient is in good body alignment.
Go to the bathroom and empty out the bedpan into the toilet and clean and dry the bedpan and store in the bottom shelf drawer of bedside cabinet.
Give patient call bell and say "Your call bell is right here and if you need any other help I will be here for you"
Ask: "Is there anything else I can do for you?"
(The bed is already in its lowest position)
Would you like your curtain opened? Do what patient wants and remind them to press the call button should they need anything.
Announce that you are going to wash your hands before leaving, wash hands and leave the room.
Leave the patient and go document the activity: bedpan care.
