pitocin induction question

A question came up on our LD unit and I was wondering if anyone knew the answer. Is there a standard drip concentration that is preferred by ACOG? Thanks for your help. :rolleyes:

I have read over and over D5LR SHOULD NOT be used at all unless specifically indicated. The reason being, if needed to bolus the Pitocin mix for some reason, you NEVER bolus dextrose solutions. We rarely, if ever, use D5LR except in the case of some diabetics who are NPO and may need it. We can never bolus D5LR in women of childbearing age as it can cause brain swelling, irreversible brain damage and even death! Not to mention, exposing the fetus to unnecessary dextrose prior to delivery.

So those using D5, what do you do if you need a pitocin bolus after delivery?

we mix our pit in LR, and run it with d5lr at 125/hr, both on the pump. For those of you that mix your pit 10 or 20 in 1000mL, Do you all run a maintenance solution with your pit? Since we mix our pit to a 1:1 solution (1mu/min=1mL/hr on the pump) the fluid volume the pt is getting from the pit solution itself is small.

We do run LR as a maintanence solution at 125cc/hr. The total fluid load on anyone is not to exceed 150cc/hr unless special conditions exist.

for those using D5LR, when you need to bolus pitocin after delivery, how do you do it? Or do you just give it IM?

we usually let the rest of the IV LR with pit run free .. if something goes really wrong we use methergin or pit IM..
