Patho for pulmonary hypertension

I have to write a careplan for pulmonary hypertension. Can someone help, I cant find what i'm looking for on the internet. I need patho and nursing problem and interventions.

the physician articles and entries in the online merck manual will almost always have the pathophysiology of medical diseases. they will also include the causes, signs and symptoms. you need that information in order to assist you with the determination of your nursing problems. these websites will give you the pathophysiology of pulmonary hypertension. please note that there is primary and secondary pulmonary hypertension and it is closely associated with heart failure and problems with cardiac output.

the nursing problems are based upon the abnormal assessment data (signs and symptoms) that the patient has. the signs and symptoms become the evidence that support and prove the nursing problems (nursing diagnoses) that are present. the nursing interventions are aimed at treating the signs and symptoms of the nursing problems. this is all covered in the thread help with care plans (https://allnurses.com/general-nursing-student/help-care-plans-286986.html). you need to follow the steps of the nursing process in the sequence that they occur to write a care plan.
