Passed nclex- reapply in nevada

Hi all, I passed my NCLEX in August of 2013. I just received my SSN and I'm now ready to activate my license. I was told that I need to reapply again. Which form do I need to use?

Is it the Application for Initial License Form- Licensure by Examination?

Please advise.

Seems to me that contacting the Nevada State BoN (the same place that told you that you needed to apply again) and asking them this question would result in an accurate answer, no? Did you ask them which form to use?

Oh I most definitely did. The operators just doesnt seem to know the answer. They keep on transferring me to another dept which either ends with no answer and voicemail. Been trying for weeks now. If all else fails, I always try to ask on forums and learn from othee people's experiences. I was just wondering if someone has ever been in the same boat.

Since this is your initial license by examination that would be the form to complete and indicate you previously applied and passed NCLEX-RN. You can try emailing NV BoN

I find it strange that nevada require you to resubmit application as usually we only see California where application has to be resubmitted once you have a US SSN. I think you really need to speak to someone at the BON.
