I volunteer at a free clinic and we see a lot of immigrants with little English ability, so I'm always on the look out for patient education materials in foreign languages. I hit the mother lode this evening and wanted to share the link:
http://www.healthinfotranslations.org/languages.php
The translations are really simple and easy for a lay person to understand. If anyone has any other sites, I'd love to know about them!
