Nursing Student: Question on Outcomes Management?

Hello Allnurses!

I'm currently a nursing student and am wondering if you could describe to me some practical ways that Outcomes Management is used day to day. I would also be interested to know about the chain reaction that has to occur in order to make a proposed outcome a reality.
