Hi

i have pt. in 27 years old, male, with Quderplegia due to Electrical shok , Bed reden , Gastrostomy , tude feeding , unconscioss and disorianted , Trachostomy was removed from 5 days , Grand mal , condom fixed ...........:imbar :stone

thats is the case :

what is the nursing diagnosis and the intervensions we can do for this pt. ????

whoa.... i hope acuity is considered when they make the assignment coz that is a handful. back to your question. just to name a few, risk for aspiration, risk for injury, impaired airway, potential for infection, skin breakdown and all the deficits you can think of would apply for your pt. if u have a pre-printed care-plan drawer, pull out one of each and stick it in his clip board.(LOL), and make out one more " potential for nurse burnout ". good luck.

You'll need to write the actual nursing diagnoses, but I can give you an idea of some things to consider.

cardiovascular problems due to the electric shock

altered skin, bowel, bladder, respiratory, thermoregulatory functions

hypoxemia

potential for pneumonia

high risk of dysreflexia (and the effects of it on various body systems)

paralytic ileus

GI bleeding (very common in new quads)

urinary retention

thrombophlebitis

wound infection

skin breakdown

safety issues (for confusion and seizures)

role disturbance and uncertain future (may focus more toward family)

Also take a look at the patient's list of medications to get an idea of what the doctor is already treating or supplying prophylaxis for.

It sounds a little bit like you want someone else to do your work for you and that is why you aren't getting many replies.

Why don't you post what YOU have worked up so far along with specific questions about certain dx or interventions

just a thought
