what's causing the anisocoria? that may give you a place to start -- if disease process or injury consider if the patient is having any other problems associated with the disease process...

is the patient having any trouble seeing or problem with depth perception? if so then consider tripping hazards..

First. Tell me....what is anisocoria? Does anisocoria always require nursing intervention?

Uhh... well there's really nothing you do to "manage" anisocoria. It just is. If you have a neuro patient with baseline equal pupils who all of a sudden develops anisocoria, there's a problem but there are plenty of people who walk around every day with anisocoria and no issue whatsoever. Something like 10-20% of healthy people have anisocoria. I had a colleague who never noticed hers until one of our neurosurgeons pointed it out to her.
