I struggle so much with coming up with nursing interventions. My diagnoses are
#1 Risk for hemorrhage r/t cesarean
section and uterine involution
#2 Acute Pain r/t cesarean section AEB
dependence on pain medication and
tenderness of abdomen to touch
Any ideas, ASAP, would be greatly appreciated. I already have 7 for #2 and 4 for #1... but I need way more (it's for my fun concept map care plan thing... fun stuff! :zzzzz ) Thanks!!

On the left side is a tab for care plans. You can select which care plan you want to see and it will give you lots of ideas for goals, interventions, etc... You can look at several care plans to come up with goals and interventions for 1. That's usually what I do.

I have a book to help with this: Nursing Interventions Classification by McCloskey & Bulechek. I also have the NOC book. It makes careplans a snap!

Does the NOC book have the Likert scales written out for each outcome?
