nursing dx question

I cant figure out how to write this DX.. its Knowledge Deficit about postpartum self care.. plz help :wink2:

deficient knowledge of postpartum self-care r/t [lack of resources, access to misleading information, not being familiar with available resources of information, cognitive inability or limitation, apathy to learning] aeb [patients verbalizations of concern, observation of patient poor performance of self-care, hesitancy in performing self-care needed at discharge].

remember that whatever you chose as your "related to" item(s), or cause(s), of the knowledge deficit must be supported by the aeb evidence. your aeb statements are the supporting evidence that you have that she is lacking in knowledge and needs educating. so, what you have to support that, heard her say, or observed her doing are going to be your supporting evidence.
