I DONT HAVE A WEBSITE FOR YOU, BUT I HAVE A BOOK THAT WE HAVE USED THIS SEMESTER CALLED NURSING DIAGNOSIS HAND BOOK A GUIDE TO PLANNING CARE AND IT IS EXECELLENT!!! IT HAS NIC AND NOC ALSO (DONT KNOW IF YOU ARE FAM. WITH THOSE TERMS NIC ARE NURSING INTERVENTIONS AND NOC ARE EXPECTED OUTCOMES) THE BOOK IS BY ACKLEY AND LADWIG. GOOD LUCK:)

I also think Nursing Diagnosis Handbook by Ackley is great! You can look up a medical condition and see what nursing diagnosis goes with it. For example,

Ear Ache- (1) Distrubed sensory perceptions auditory r/t altered sensory reception, transmission and integration (2) Pain r/t trauma, edema

Then you can look up each nursing diagnosis and get all the NOCs and NICs with rationales. Well worth the $30.
