Hello all....I must do a concept map,which is due on 4/30. My patient has a diagnoses of HTN, artery diease, and Rectal cancer, and has a ilestomy he had surgery on 4/2/08........I dont know where to start...any ideas what would be some good nursing diagnoses for this patient????

Subjective --> What the patient says

Objective --> What you directly observed, this includes lab values, physical assessment findings, vital signs etc.

Then we can go from there!

You start by sitting down and going through the assessment data that you collected. You are looking for abnormal data. You also need to look up the pathophysiology of the HTN, artery disease, rectal cancer and information about ileostomies and their care.

You follow the steps of the nursing process in putting all the information together for your concept map.
