Job application after frequent job changes/termination

Recently terminated during initial 90 day employment period from a Dallas

hospital that uses Group One as an employment reference. Was a

hospital based telephone triage job which was completely new to me.

Although told by trainer I was doing a good job and the training would take

10 to 12 weeks, was let go by manager after about 9 and 1/2 weeks as he

said the training should only take 4 to 6 weeks. This happened at the same time that the call center's 2009 budget was cut and our orientation hours were reduced and the shift I was hired for would be changed post orientation resulting in a wage loss.

Held 2 jobs (2 months and 5 months) as well in past year. Was transitioning from inpatient/on call operating room to outpatient/no call operating room to leaving the operating room because it is too physical demanding for this older nurse.

Need advise for applying for office/clinic/urgent care positions. How can I get an interview when I am struggling to fill out applications due to 3 jobs in 1 year with termination from the last one?

How do I answer questions like what was my reason for leaving? May we contact this job? Supervisor's name? Reason for being discharged from any employment?

Have been a nurse for 30 years. Still have 10 to 15 years in workforce.

Need advice, especially from nurses in Dallas area, who have experience with Group One termination. :confused:

Sorry I cant provide your response to your question, but empathy. I am now in a similar maybe worse place. I am in the Dallas area & recently terminated frim my first & only nursing job. I thought everyone used GroupOne. My report shows involuntary suspension d/t unsatisfactory job performance. How do move on from that? At least u have a lot years of experience, I have less than one with termination...It's been a while since your post, how's it going for you since your separation?
