I have a care plan that is due tomorrow and I'm not quit sure how I should right the nursing diagnosis. My patient has generalized weakness, is malnutrition, and has hypothyroidism.
I was think for them to be
Activity intoloerence r/t generalized weakness aeb
Altered nutrition r/t inadequate dietary intake aeb
Risk for falls r/t decreased mobility

The AEB is something that you see in the patient. For nutrition it could be how much you see that they eat. Or it can mean the labs that show it too
