This is not news as it is something we have all experienced for a long time. Dealing with EHR platforms that are complicated and not user friendly has made our jobs more difficult.

Yeah. Every single time we suction. One of the agencies has this done very simply (state why, check off where, check off whether or not it was deep suctioning, and "Lg, Med, or Sm"). The other agency wants why suctioned, where, how deep, describe secretions, whether airway cleared, and how tolerated. Every time.
Would it not make more sense to say this just once an hour? Or less?
The agencies are afraid that the insurance will drop the patient if the charting does not prove that the patient does, indeed, need skilled nursing care.
