Nrsg. Abbreviations ????

Hi everyone,

I have to write up a short paper on patients Hx, Diagnoses and Treatments.

The problem I am having is that I do not know what some of the abbreviations mean in regards to treatments that I have found on the record.

A couple I have found on the internet but there are a few that remain that I still cannot figure out and can't find anywhere.

If you have any idea what some of these mean please let me know, I would greatly appreciate it.

Padded SR's increase x2 to A with self positioning R foam position wedge against R BedRail w LB

(I'm thinking something like "padded ----- increased to 2 to A (aid) with self positioning, R (Right) foam position wedge against R (Right) Bed Rail with -----)

I have know idea what a SR and LB are. Also, the A and the R had little circles around them, my translation may be wrong.

24" wide w/c with 24x24" gel cushion with foam insert beneath, B Leg Rest & B custom foot box and SRSB

(I'm thinking 24" wide w/c (wheel chair) with 24x24" gel cushion with foam insert beneath, B (bed) leg rest and B(Bed) custom foot box and ------)

I am not sure if my use of Bed for B is right and don't have a clue as to what an SRSB is.

If you have any thoughts or have seen these used before please let me know.

Hospitals usually have a list of approved abbreviations. I can almost guarantee you that all of those are not in the approved list. R with a circle around it usually means right. A with a circle would mean assist. SR is side rails, LB is lower back? Just guessing on some of it, just like you! :uhoh3:

B usually means bilateral. Don't know what the last one could be?

Colleen, I 'm not sure what SRSB is off the top of my head either, but you could try posting this in the rehab forum as these sound like orders rehab nurses might typically encounter. Good luck.

Translation: Padded siderails up (an arrow pointing up, right?) x2 (both sides up)to assist with self positioning. Right foam position wedge against right bedrail with LB (that one is a new one for me.)
