Hospitals usually have a list of approved abbreviations. I can almost guarantee you that all of those are not in the approved list. R with a circle around it usually means right. A with a circle would mean assist. SR is side rails, LB is lower back? Just guessing on some of it, just like you!

Translation: Padded siderails up (an arrow pointing up, right?) x2 (both sides up)to assist with self positioning. Right foam position wedge against right bedrail with LB (that one is a new one for me.)
24" wide wheelchair with24x24" gel cushion with foam insert beneath, bilateral leg rest and bilateral custom foot box and self-releasing seat belt.

Hi everyone,
I have to write up a short paper on patients Hx, Diagnoses and Treatments.
The problem I am having is that I do not know what some of the abbreviations mean in regards to treatments that I have found on the record.
A couple I have found on the internet but there are a few that remain that I still cannot figure out and can't find anywhere.
If you have any idea what some of these mean please let me know, I would greatly appreciate it.

(I'm thinking something like "padded ----- increased to 2 to A (aid) with self positioning, R (Right) foam position wedge against R (Right) Bed Rail with -----)
I have know idea what a SR and LB are. Also, the A and the R had little circles around them, my translation may be wrong.

I am not sure if my use of Bed for B is right and don't have a clue as to what an SRSB is.
If you have any thoughts or have seen these used before please let me know.
