NP specialty with the most patient time?

Hi there,

I've been working in primary care for several years now and find myself stressed by the short amount of time allotted to see each patient. I know this is the nature of primary care, but I don't feel that I can give the best care to the patient in this format. I'm looking to eventually move into a specialty, though I'm not sure which. Does anyone happen to know which specialties or practices allow for the most amount of dedicated patient time? Thanks in advance for any advice!

I work in pulmonology and my new patients slots are 1 hour and established patients are 30 minutes. 

I am in nephrology where I am 50% office and 50% dialysis.  In the office I am allotted 40 minutes each patient.  I have 10 patients max per day.  As someone referred that specialty focuses on theirs alone and does not care about the rest, I will have to wholeheartedly disagree.  That said, I do know my specialty well and the patient who is there for a kidney problem deserves that I give them this focus. I had a derm issue today, ordered some topical, pretty sure will be fine, but patient awake at night with pruritus, how can I not?  I did keep this brief, end of this discussion said he needs to see PCP if not resolved in week, then on we went into the quiet kidneys...My team is 7 physicians and 3 NPs, owned and run by the nephrologists.  It makes a difference not being directly associated with a hospital or large corporation and I am very grateful for my position.

I was the one that posted about the "stay in your lane" attitude I got in my outpatient rotations for clinicals. I'm glad to hear you say this is not always the case. In all three of my experiences, the MDs did not address anything outside of the specific issue for the patient visit. One even wrote in my evaluation "____ should definitely consider hospital medicine or generalized practice. Her desire to address multiple issues in a short time frame could be a challenge for transition into a specialty clinic." That made me laugh because I really didn't understand how some of the things I tried to address weren't directly related or at least impacted the patient's care. Oh well, at least I know where I shouldn't apply for work.
