hi i would like to ask

regarding FORM 1 for New York Application

what will i do if i really dont know the exact date i entered primary, secondary school,

but i know the month and year? what will i do?

what does "please initial" means?

if i will have it notarize at US embassy in Manila, should i have an appointment schedule?

how much does it cost? is it per stamp?

i'm also completing my CVS application online, when i entered the month and year in the dates of my attendance, it automatically entered the first day of the month, will that conflict with my records? or is it understood that it was done automatically by the service online?

Initial means initial so if your name is Fred Bloggs then initial would be FB.

Been a few years since I completed CVS and can't remember what I wrote. You could try searching for other threads discussing NY otherwise I am sure some members who have recently completed CVS will be able to help

Hi I also have a question about form 1, item 18. It asks "do you now hold or have ever held a license or certificate to practice any profession in any jurisdiction?" Does jurisdiction mean a US state or a country? I do not have a license in any US state, but I do have a license in our country. The question also asks for a Form 3 to be filled up by the licensing body who issued the license. However, form 3 pertains to the NCLEX test result which does not apply in my country. The term jurisdiction is quite confusing, and it would be nice if anyone could clarify things. Thank you.

Jurisdiction as I read it is for any country and I am sure I filled this form or something similar for the NMC in the UK, they will fill something out appropriately.

hi... i have a follow-up question here. in CVS... do i have to include that i have passed NCLEX in California?? or CVS is just mainly for licenses i acquired in my country. I still don't have SSN wlhich is required to be licensed in California. When will i indicate i have passed nclex already, is it during my application in New York?? thank you very much
