(transcript validation form) accdg to cfgns they have sent the document but my school says that they haven't receive any. (sighs) but my friend who also took the same service said that his verification process took 107 days to be exact to be processed and his papers are now at BON NY

hi i would like to ask regarding FORM 1 for New York Application what will i do if i really dont know the exact date i entered primary, secondary school, but i know the month and year? what will i do? Attendance from: ________ / ________ / ________ to ________ / ________ / ________ mo. day yr. mo. day yr. on the education program review part what does "please initial" means? I give permission to the New York State Education Department to release my examination results to my professional school for the confidential purposes of program review and institution research and planning. I may rescind this authority at any time by notifying the Division of Professional Licensing Services in writing. ? Yes ? No Please initial: __________________________ if i will have it notarize at US embassy in Manila, should i have an appointment schedule? how much does it cost? is it per stamp? i'm also completing my CVS application online, when i entered the month and year in the dates of my attendance, it automatically entered the first day of the month, will that conflict with my records? or is it understood that it was done automatically by the service online? thank you so much for those who will help me! thank you in advance!

hi! i'm a 2009 BSN degree holder in the Philippines, i've passed the licensure exams in the Phils and just took oath last month as a registered nurse in the Phils. But since i'm a permanent resident (immigrant) already in the US. i intend to seek opportunities in the US, particularly New York. i've read from the posts and from the net that i need to have my credential verified by CGFNS thru their CVS service before i can apply for licensure for New York and NCLEX exam. my questions are: what forms should i pass to CGFNS for application for their verification service? how many copies should i pass to CGFNS? where will i mail the forms and where shall i pay the appropriate fees? do i still need a form for in order for me to pay? because i saw a creadit card form on cfgns.org website. should i also include that with my application forms for verification service? thank you! hope to hear from you guys!
