Yes, you are required to give your SSN when completing the application packet.

There are questions on the application for licensure regarding if one has been convicted of a crime or offense, and or pleaded no contest. An affirmative answer requires submission of a letter giving details along with court documents.

BON will do their own criminal background check against state and IIRC FBI database. If something comes up not included on the application packet the applicant will be contacted. Upon disclosure of any criminal background as well (on the application packet) the applicant will be contacted.

Keep in mind just obtaining a license from NYS is not the end of things background wise. All facilities and or services will require their own criminal background check some with fingerprints.

Bottom line is if there is something in your past you aren't too proud off best tell the truth and shame the devil. If the NYS BON finds out on their own then or later you'll be in far worse shape than if you fessed up front.

The BoN specifies where to get fingerprints/background first. They usually are only valid 6 months. CGFNS cvs for NY averages 6 months processing so it may be a waste doing fingerprints/background first as you will likely have to pay again to repeat the background check.
