New resources for Chronic Health Conditions

Good morning- I want to share a link to our Missouri Department of Health School Health website. It has 2 new resources- and lots of good ones previously. The new ones are SN CHAT (School Nurse Chronic Health Assessment Tool). It is a wonderful resource.

Also on the same page- is the link to click on for the Emergency Action Plans. They are fillable forms and are also included in the SN CHAT.

Thanks for  the SN CHAT and emergency action plans link.  Will adapt for a few children at Summer camp next year as only 1 of the asthmatics had action plan submitted by parents, NONE for our 4 diabetics.

WOW!!  I am very impressed with the School Health Program in MO- like really impressed!!  This website is something every school district should strive for.  I will definitely be using this as a resource in my practice.  Thanks for sharing.

You are very welcome. We are a pretty cohesive group that help and share with each other to the benefit of our kids and school communities as a whole. Our State School Nurse Consultant and her Staff are amazing on so many levels.
