I can't take it anymore!!! I had a break down today, and I just had enough. I had a family crisis and I don't even live close in the area that I work. I want to work closer to home and be with my family.
What do I say to my boss to let her know that this is final?:lol_hitti How should I say it? Anyone, please, please help. Thank you.

Is there anyway to sit down and try and talk to your boss about what is going on? Can you possibly take a little time off to resolve the family crisis? I would just suggest being very honest with her about what is going on and what you need. You owe at least that much to her and you have really no idea what she'll be able to do to help you through the crisis.
Also, be prepared to work through the appropriate notice and time frame which might be as long as 4 weeks. I know it might be tempting to just bail, but don't burn any bridges because you never know when and if you might need this facility again in your work life.
I have more benefits helping out my family and being near them than work. My job is hours away from my family. I think I am prepared to accept working as long as four weeks prior to resigning.
I spoke to my boss about the situation over the phone. she said it might get better tomorrow and then I could work the next day. Or I could take the rest of the week off and come back next week.
But I want her to know that I am serious about this. How do I do it?

Can you clarify what you mean by you "want you manager to know you are serious"? It sounds like you've already made up your mind about quitting. In that case, write a letter of resignation, and state your last day of work in it. It's that simple. Is she trying to talk you into staying? If so, the words "I'm sorry, but this is simply not working for me" work pretty good. Don't give in to offers if she makes any. Cause if you take them, you become obligated again.
I have made my mind. I've talked to the nurse manager over the phone and has told me not to resign.
I don't know how many weeks to give her because she just doesn't want me to go ahead with the process. So it looks as though I might have to bail out..
Since I have worked there a short time, I can't imagine what it would be like to go through the interview process for a new job closer to my family. This crisis was unsuspected and I just have to be here for my family.

Give two weeks, or whatever is stipulated in your employment contract. If they need a reason, tell them it you need to be closer to your family.
In my experience, work will always be thereand I would never sacrifece time with loved ones over a job.
Don't feel guilty or pressured from your manager. I am sure that your are a great employee and they don't want to lose you, but that is their problem.

I agree with Wolfpackred...love ones should always come first...main reason I went into nursing was so I could spend more time with family...lol, which at this pt in time has not really happened...life is too short to not be happy most of the time...good luck in your decision...I'm sure mgrs are use to losing employees...I have classmates that have been through quite a few jobs in their 1st year...
