hello everyone
This is my first time posting on this site so Iwould like to say hello to everyone. I have a dilemma i work in the worst hospital in boston . Please feel free to take guesses . Two days ago I had this pt who brought her own meds to the hospital and the doctor wrote an order for her to receive her own meds. Pharmacy was suppose to put the med information in the pyxis when I go to the pyxis to give her the med which is a normal medical nothing major, the pyxis gives me a narcotic instead, without count , just by pressing the other medication name. I double check both medication and then I check her MAR to see if she has orders for narcotics NO. Then I go to the pt she is alert and ox3 and ask her if she have ever taken the narcotic that the pyxis gave me she said no. I then called another nurse to double check. she does and realizes the same thing. so then I called the supervisor. I explained it to her she puts in the name of the medication and the pyxis once again gives her a narcotic without count. she is shocked and realize the pharmacy made a big error This supervisor said she will write an incident report.. What should I do to cover myself. should write an incident report of my own to cover myself. thenext day at work I did not see the supervisor. I do not trust anyone in this hospital, so I want to cover myself as well and not rely on the supervisor word only.
please give some advice

I would write a statement and have the nurse manager sign it. Things can get a bit messy and interpretations lost when there is not a paper trail. Is this hospital union or non union?

File your own incident report. Put in as much detail as you can. Cover your butt because no one else is going to.

definitely a detailed incident report (make a copy for yourself);
and notify pharmacy!
