If you have been terminated from a facility, do you need to indicate that on your resume? What should you put down in that case? When a potential employer asks you for your reason for leaving a job either on an application or during an interview, what should you say or write down?

That's not appropriate to include in the resume.
As to what to write on an app or otherwise, that would depend on the reason for the termination.

i would consider two things- were you terminated (or asked to resign), and how long were you employed there? also, is this a nursing position, or a previous career? i had the unfortunate experience of being asked to resign from a position- i was only there two months, and they didn't feel i was 'cut out' for that specialty (forget about the fact that their orientation s****d, and did not adequately prepare me, but i'm not bitter :angryfire :angryfire , but i digress).
how did you leave it w/ your employer? really bad terms? i don't thinking this is black and white, without knowing a little more. but definately don't include on your resume.
