We are looking for some kind of nifty sheet or form that we can use with our new grads to help them learn to give report in a thorough and timely fashion...anyone out there have one so we don't have to reinvent the wheel? Thanks a million in advance.

I once had a report sheet that listed pt. name, age, diagnosis, physicians, diet, allergies, there was a place for medical history, then each major body system.
There was another section for labs and we always reviewed orders from the last chart check. That seemed to be a comprehensive report. This was ICU btw.

We have a sheet like that for our new people. It has a space for meds, feeds, labs, etc.
