Need help with nursing diagnosis

I'm doing a care plan on a postpartum patient who had someone bring her medication from outside the hospital and midway through the shift had a dramatic change in LOC due to OD. She was hospitalized 3 times during her pregnancy for similar behavior. She also has a history of bulimia but I didn't get a chance to ask her anything about it before she started talking nonsense.

I need one more diagnosis. Also, I feel like I should change some of the ones I have so I don't have quite as many "risk for" diagnosis.

Here's what I have for my nursing diagnosis:

  1. Confusion r/t drug abuse AEB incoherent speech
  2. Risk for injury r/t altered LOC
  3. Risk for infection r/t knowledge deficit (she was putting tissue inside her lady parts)
  4. Risk for infection r/t operative procedure
  5. Risk for injury r/t operative procedure
  6. Knowledge deficit r/t activity/exercise restrictions AEB OOB and pacing 24 hours post-op
  7. Risk for self mutilation r/t history of self-injurious behavior (she cutts herself)
  8. Risk for impaired parenting r/t altered LOC
  9. Anxiety r/t fetal well being AEB patient questioning when she will be able to see her son

I appreciate any help you can offer!

I just remembered she's GSB+

It's not proper to have Nursing Diagnosis related to another nursing diagnosis like you have with "Risk for infection r/t knowledge deficit."

You also might want to look at Risk Prone Health Behavior.

I changed that diagnosis to knowledge deficit r/t lack of exposure to teaching AEB placement to forgein object in lady parts during puerperium period.

"Risk prone health behavior" is that a NANDA diagnosis?

Wouldn't have mentioned it if it wasn't.;)

How is oob 24 hours post birth bad? Even with surgery they yank us out of bed then.

You're right. They do stress ambulation but with this patient they were afraid she would do too much and would be in severe pain once her high wore off. I guess I should reword it. Thanks!

Ah. Ya learn something new every day.

How about ineffective family therapeutic regimen management since family did bring meds from the home...
