I am planning to take the NCLEX-RN exam for California.
im from the philippines..
Since im applying on my own, i just need a few questions
to help me fill up the blanks in the forms to avoid any errors.
APPLICATION FOR LICENSURE BY EXAMINATION form:
1.) COLOR OF EYES- should it be abbreviated like BRN (brown) or the whole word? since in the fingerprint card it should be shortbut, but i dont know how i should put it in the application form.
2.) PRIMARY LANGUAGE- im a FILIPINO.. so what should i put in here, should it be FILIPINO or TAGALOG/ENGLISH?
3.) PHOTO- how should i paste it? glue? scotch tape?
4.) SOCIAL SECURITY NUMBER- im a foreign nurse, should i just leave it blank including the fingerprint card SSN, or should i put NOT APPLICABLE?
REQUEST FOR TRANSCRIPT form (to be completed by applicant)
1.) YEARS ATTENDED - all in all i spent 5 years in Nursing course,
but i took my first year nursing at a different school and it is asking for YEARS ATTENDED next to the name of my school.
I took 4 years in the school i graduated from. should i put 5 years including the different school or only 4 years where i graduated?
LAST question:
IS IT OKAY TO PASS 2 FINGERPRINT CARDS
WITH MY APPLICATION BECAUSE I HEARD IT
SHOULD BE ONE EACH BUT I JUST WANT TO MAKE
SURE THEY WONT REJECT THE OTHER ONE..?
DO THEY CHARGE DOUBLE FOR FINGERPRINT CARD
PROCESSING IF I SEND 2 FINGERPRINT CARD?

You are applying for licensure as an RN in the state that you choose. You are not applying for the NCLEX exam, that is impossible to do.
Thread moved to the International Forum since it is specific to working in the US. And there are already multiple threads on this very same topic. Please take the time to do a search for the information. It is also included with the instructions that come for preparing the application for licensure.

answers to your questions:
1) color of eyes: BRN for brown, BLK for black
2) primary language: FILIPINO
3) paste or glue on four sides
4) social security, write NONE
request for transcript form
1) number of years in the specific school where you attended, i believe they have additional spaces in case you attended two or more schools.
re fingerprint card: if they sent you one, just send one, if two then send two. no need to add if they only require one fingerprint card.

is it ok if my finger print card has erasures? i wrote my weight in kilograms instead of pounds... and also i wrote my citizen even there was no 'x' marked on it .

I think if there are errors and erasures then it probably will not be accepted. If error with just a line through and then correct may be but otherwise I think you will have issues

it might have errors on if it has erasures.
the picture would have to be taped, as i remember that is what i did. i think there's a not detailing how the picture should be attached.
