In my peds rotation, I had an infant who was on Orapred because she has a condition that causes bone marrow failure. The indications of this drug are for inflammation and immunosuppression and I am having trouble evaluating its effectiveness. Should I just put does not show signs of infection? Any help would be appreciated!

Is the patient receiving chemotherapy? an autoimmune disorder? What is the cause of the bone marrow failure to produce cells?
Assessing the effectiveness...well of the top of my dome i'm thinking if there is a problem of cell production I would check the progression of the patients labs, particularly WBC, RBC, Pltts. I mean if the patient receives transfusions it might be difficult to determine whether the drug is having its effect or whether it's really the action of the xfusion. Perhaps getting a lab for the production amount for immature WBC's might help... just grabbin straws :)
If your patient has nausea and vomiting while receiving chemo (if they are), prednisolone is listed as an adjunct to prevent that - so if that helps them, you've got something else to write about.

An evaluation is similar to the evaluation step of the nursing process. Evaluation is another assessment.
