Need Charting Help Please

we use a flowsheet for our skilled/subacute charting. it has the check-off stuff on front for all three shifts, pain scale, place for vitals, etc...and place for nurses notes on the back(skilled) the sub-acute one is just a check-off, but there are more things on it like suctioning, vent settings and so forth. for our long term residents we have monthly summaries and just regular day to day nurses notes.

hey deb! thanks! is there any way i could have a copy of that flowsheet? i have tried to find flowsheets online that i could adapt for us but have had no luck.

I usually made our flowsheets where I worked off of the spreadsheet on the computer. If you call Briggs, and request a flowsheet from their catolog, then they will send you an example for free. You can make up your own, using theirs as a guide. You can even send in your flowsheet to them, and they can make it especially made from your own ideas.

At the retirement home where I work, we use what's called a "unit daily planner" which may be similar to what you're describing? Perhaps? *lol*

What it is is basically two page document with areas for each shift to chart on noteworthy events, sign off on who was working that shift (granted, we only have 2 staff on at a time), make notes, chart assessments for a few patients.

oh, pain scale, of course...forgot that.....oh, and trach inner cannula changes, dress. changes, alll kinds of tubing changes!....and peg site notations! hey, there is a lot of stuff! lol

i just ordered some catalogs from briggs and will call them tomorrow. i am still looking tho! thanks for your help! lol my don said he didnt want to "reinvent the wheel" and has me on this wild goose chase. i am thinking maybe i should just come up with one and patent it! i could post it...we could all work on it....lol

I have never worked at a TN NH that used flow sheets. We've alway did regular nurses notes on everyone including skilled and subacute. We chart on skilled and medicare daily. The rest are weekly. We do have a flow sheet for monthly, but that is on top of the written nurses notes. I'm sure the flow sheets would be welcomed if they decreased the number of narratives.

that is exactly what we are trying to do! work smarter....not harder. i am still looking tho lol
