Good Morning!
Thank you for reading and for your time. I have some questions regarding my situation.
I passed my NCLEX December of 2009, I didn't have my SSN that time so I wasn't able to get my license. I just got my SSN this year and I want to know what are the requirements that I need to submit together with my SSN and the Passing letter.
Also I was wondering if I need to submit my marriage certificate because I am married now (married name) and I was still single when I took and passed my NCLEX. Do I need to put my married name in my application for licensure or my maiden name?

You don't have much time. Your file expired after 3 years. At the 8 year mark you need to start all over and retake the NCLEX.
You need to provide your marriage certificate that shows both names to file a name change with the BoN

CA is backed up by months. Be prepared they may choose to reassess all of your credentials since its been 7 years

as fas as i know the 8 year retake is for those who have permanent license already and not to those who passed the nclex.
Eight year retake is not only those who were licensed. Anyone who has not worked as a licensed US nurse since passing the NCLEX in CA is subject to being required to retake the NCLEX-RN. The NCLEX has updated three times since you took the test including elevated passing standards combined with a stricter evaluation of IENs by BRN evaluators to combat fraud plus enforcement of concurrency of theory and clinical practice.
They may just issue a license even though your file was destroyed 4+ years ago. They may choose to reevaluate your credentials. Be prepared for either option
i see, thanks again for the info, very helpful, i just hope mine wont have any problem just like those who took nclex2007 and submitted their application and got their license last year.
There are no guarantees either way. Hope for the best, prepare for the worst

Every year, the NCLEX-RN exam becomes more tough to pass.
Every 3 years it's updated. Not annually
