If the precautions weren't maintained, then you shouldn't have signed it. If your facilities policy is that they aren't in the dining room, the policy was broken. (Incident report time.)
Honesty in charting, even if it makes the institution or yourself look bad, is always the best policy.
Perhaps there should be a policy in place what to do when the floors are being waxed.

We have two residents on MRSA sputum precautions. Today they were both in the dining room eating with other residents . I refused to sign the spot on the MAR where it says "MRSA precautions maintained " . I was the med nurse today ,the charge nurse when I brought this up said " Well their floors are getting waxed what do you want me to do ?" UGHHHHHH What would you have done?
