15 month old getting 750mg Rocephin

Ok, so I have asked my friend who has been a pediatric nurse for a while if I am ok, but I still want some clarity. I work in a pediatric doctors office. Today I had a 15 month old that needed rocephin 750mg IM. I drew it up and got all my stuff together. I had given rocephin quite a few times, just not to someone this young (I have given it to bigger kids, like 8 years old and older) I always gave it in their buttocks. So this time I did the same thing. I split it up and gave two injections. Everything went fine, of course he cried, but that was to be expected, and I asked the parents to just wait about 15 minutes so we can monitor him since this was his first time ever receiving the abx. As I was charting on EMR, the nurse who will be giving him the second dose of rocephin tomorrow asked if I split it up, and if "I gave it in both the thighs" I kind of got nervous that this is the only place I should have given it? He did not have any reaction, was a some-what happy boy when leaving. Did I do anything wrong by choosing this site? I did good technique, so I know the injection was correct, but I just want some input from others. Thanks for advise!

I have issues with the use of Lidocaine. First, Rocephin is already somewhat viscous, this increases with the Lidocaine. Second, anyone who has had Lido injected should know how badly it burns and stings. It takes a few minutes for Lido to numb an area. I don't believe the mixing provides any benefit to the kiddos during the injection. Just gives them a numb lump that will be sore later. I figure, get it over with and work it out, just like all the others.

What do you use as a diluent then?

Per the manufacturer's recommendations it can be diluted with sterile water. One of my home care kids was recently home on IM Ceftriaxone as his CVL was infected and they weren't able to place a PICC. The pharmacy supplied bottles of sterile water for reconstitution.

I suppose that's another thing we don't have in common as we pull ours in babies. I find Rocephin to be like syrup whether its reconstituted with lidocaine or water. I hate IM. It is so painful. If at all possible, we put in a PIV.
