MMQ and 15 days

I am, among other things, the MMQ nurse for my facility. We just had our turnaround. I claimed a skilled procedure because the gentleman had a treatment which,although had many changes, was done for more than 25 days. The surveyor disallowed the points because she didn't count 15 days. I have no corporate support and am, obviously, clueless about this, but I thought I could claim a skilled treatment if it was done for 15 days or more. Anyone know????

Did you claim the skilled procedure in # 10 or # 12 since the documentation for each is different

Yes, 15 days documentation, but other things must back it up. Say, on a treatment for O2, was it merely signed off...or was there a dx, a written shift saturation note written, the outcome of such treatment, inhalation treatments...was that same O2 therapy claimed twice, skilled observation and skilled treatment others, you may only claim it once...that was a small example but need you to be more specific to answer it properly.

The main thing, is your fifteen day documentation merely a signature, or is it backed by the MDS, a good note with outcomes, is it an accepted part of the DX, is it really necessary etc...please elaborate a little further. Peace!
