I work with 90% of my residents having some form of dementia, MID, Alzheimer's, Lewy bodie etc.(I'm in UK so our charting is different)We try to assess what each person's 'normal' is.This can take a good while till we get to know them.If ,for instance,they use 'key words' for certain things.i.e one resident says 'Baby cakes' when she wants the toilet.Once you assess what is their normal then you can often spot when something is not usual for them.

Memory Care definition? LPN's duties? How to document for pt with dementia?
I got an idea that Memory Care is for those with Alzheimer's and dementia. But I dont know how their health condition is like comparing to those inAlzheimer's unit in SNF. So Why is it called different and/or what is the difference?
Besides passing meds, what else do LPN do in a Memory Care facility to the pt?
How is documentation for Memory Care pt, or pt with dementia in general, like? e.g. I cannot say pt is oriented (because they have memory issues).
Any input about Memory care nursing is greatly appreciated!
