  1. Med Error

Not to beat a dead horse but I want to mention the dual sign off again. The reason it’s there is to protect from a potential med error. Personally I won’t sign off someone’s med unless I saw the vial. I personally like to show them the vial, do the...
  2. How does your unit determine who floats?

We go by per diem first, then by a rotating list. Also you can’t float until 6 months of orientation. But we are union so it’s easier to follow the system since it’s in the contract.
  3. Telemetry Monitoring

I assume your talking to me. Its a step down unit. But it’s the same for all units except the monitoring via EMR is every 4 hours. You would still post the same. ICU is every hour. Also we monitor our own tele.
  4. Telemetry Monitoring

We print a strip at start of shift and post to a monitoring sheet that goes into the chart. We document every 2hr on EMR. If there is any change in rhythm we post another strip and document. If there is a change in rhythm we also post the vitals with...
  5. New protocol - No report from ER to floor...

  7. Need help with syringe test question?

It tells you the answer on the picture of the box. 150mg/ml which means In 1ml there is 150mg. You will draw up 1ml to get the full 150mg.
  8. Need help with syringe test question?

Can you post the actual question? Because you posted the stock photo. Not every concentration on med is the same. And you said the instructor said it would be 1.5ml which isn’t possible with the concentration you are posting.
  9. Need help with syringe test question?

Did the question have a picture of the medication? if the side to be administered is 150mg. You will need to draw up 150mg from the vial. But you need to know what the concentration of the medication. For instance- If the vial says it was 150mg/ml....
  10. iv's

When I was a new grad I wanted more experience. I talked to my manager and we used an education day and I was sent to the ED and put them in for all the nurses.
  11. My First Write Up

I know it’s resolved but I’m curious on where the patient was coming from? Did you get report? I had a similar situation when I had patient transferred to my unit (I’m a step down unit). The nurse attempts to call report and the secretary stated tha...
  12. 2 year night shifter thinking about days need help!!

I was on nights before going to days. Days works better for me for my family and plus I felt I gained so much knowledge during days. I was getting bored on nights because I finished my work so early and then went and read every progress note about my...
  13. bedside report

I personally think a full a handover at the bedside is inappropriate. I do give report outside the room and then go in with the nurse after and do a quick goodbye/introduction. I like to update my board at that time, offer toileting or drink, clarify...
  14. Free teaching labor

I am currently precepting a BSN student. I don’t get paid extra but this girl is amazing. She is really advanced and takes the whole team. I need to be there for med passes, assessments, etc. She then documents and I read everything she writes and pu...
  15. Did this patient overreact?!

It's not your job to prove anything. For now give her support, only use female staff if at all possible or provide female witness if males are necessary.
