Medication administration permission

Anyone willing to share their form for permission to give Rx meds at school? Ours is old & outdated and I'm trying to rework it.

So I have a form. But I'm not also picky. If a doctor's office has their own form that has necessary info for order on it and signed off by doc, I will also get parent sign off and I'm good.

But I'm in an area with lots of medical care and some office has their own forms formatted to the local public school district's forms and I can totally work with that.
