So today was my first clinical day at st francis hospital ny. for first day it wasn't bad.my patient had a CABG.she was very cooperative.but i found myself a little lost not really lost but when i came in i asked her why she was her and she told me and we had small talks but there were times when i find myself having nothing to say. so can anyone give me some tips on that and how to manage my time since im gonna start giving meds( i'm a little terrified because i've never done it and a lot of the skills i learn at the school which were never done on a real patient i forget).Any helpful tips would be appreciated in terms of what do you do the first thing you get on the floor and get report from your nurse and so on. thank you in advance

Get report. ASK questions!
ASSESS your patient. Nothing you do makes any sense unless you assess.
Here is the time to dig into the chart...but here is what you're really going to do:
Give the am meds that are needed. Look them up, pay close attention to parameters, etc. Why are you giving them? Side effects?
Now read the chart. If you gave pain meds follow up.
Vitals q4 or q8 will change things.
Bath? Linen change? Don't forget I&O!
Otherwise, keep up with your orders, note your chart if you have to do that, give meds if they need them later.
Get a piece of paper and make a grid. Literally check things off when you do them.

when i get to the floor- i see who my nurse & tech are,
after that, i get in the room do my basic exam/assessment.
Get ready to do my 900 meds, get vitals after that, bed bath linen/change.. chart while im doing all of this stuff...Once i get ALLLLL this stuff done, i work on paperwork, or I help my peers. Day goes pretty fast if you plan it out.
Oh yeah, get a report sheet to plan out your day.

I had my first day at the hospital on moday at med surg also. Our teacher wants us to start off doing basic stuff to get comfortable doing it so we can become more efficient at it and ease new stuff in there as we go along. Were starting just doing vitals, assessment, bedbath/linen change, and charting.
so my second day was good. i gave meds.my patient looked better from the first day.she was tolerating her pain even though she said it was a 5.she walked from her room to the end of the wall with assistance of course. i did my vitals assessment. i'm suppose to do a care plan but i dont know what to have for my diagnosis since she's not really complaining of pain. her only thing is that she wants to be able to walk on her own but i just try to encourage her and tell her that she's making progress and she needs to take it one day at a time because she could have it worst. i feel like i'm suppose to put things together from classwork and into the hospital but i dont find myself doing much to be "putting things together yet" my quest is when you guys do you assessment head to toe how do you go about it...do you ask all the subjective quests for each systems?and also when people have CABG why do they use the shapneous vein and what exactly do they do with it?
