Med pass

Should a director of nursing be giving step by step directions on which residents to start with and naming other residents in the order in which a cmt is to pass her meds?

ex go to the second floor first (give three people out of 25 ) their meds 

then

go downstairs and give 3 people then go back upstairs and pull the rest out / lable the cups and put the meds in and go back downstairs and carry all those to give to residents that are just everywhere ?

I would think I'm on the cart and I will conduct my medpass in the way in which I was taught the safe way ,, oh and I'm not to look at the mar as I'm passing them 

was told to just dump them out the bag and chart when I'm done

Yeah, no. At first I thought maybe the DON was giving you advice on what residents need meds first based on preference, certain meds given before eating or maybe working around a therapy schedule and thought well, that's nice.

Then I read the post and that is all sorts of wrong. It will take longer to do a med pass correctly, honestly I worked in a SNF for 25 years and I was fast at a med pass and still could not always finish in the state expected two hour window. But then in 25 years I never, not once saw a facility cited for giving meds outside of that one hour before or one hour after window, Do your med pass correctly, follow the rights of med administration, sign them out in the MAR as you go.  What the DON is directing you to do could result in massive med errors, possible resident harm and it could put your hard earned license in jeopardy.

Agreed.  Safety first.  Place yourself in the patient's shoes.  How would you feel if you were the patient, with your current knowledge, watching someone pass meds the way you have been instructed to?  Most patients do not know the rules and regulations, but that is where our integrity comes into play.

I keep putting targets on my back by opening my mouth, however, my integrity and conscious prevents me from skipping safety rules.  It's how mistakes happen and it places someone at risk.
