I have a question.... If you have been working for Maxim Health Care for over a year and 8 months and you work 84 hours a week shouldnt you be entitled to a payed vacation, a raise, and overtime pay for working 84 hours a week?

I would get with the company and ask what policy is in place for hours worked and vacation time. This is an agency so there is flexibilty, consider if you are on contract when you want vacation or try and schedule around assignments. I would really get a clarification with the agency or your nurse manager........

Agency doesn't pay vacation. I know them.
If you did not work over 40 hours in ONE facility you short yourself out of overtime. That's a problem with agency. If it is over 40 in one location then yes and the labor dept can help settle that. Actually, just by calling or saying you will call they will pay. I had dealt with this with a contracted company for Bell Telephone over 20 years. Some guys I knew got ripped on OT. One call to Labor Dept and they were apologetic and paid ASAP.

I have always received overtime for anything over 40 hours while working agency. Even if the hours were at different facilities. Never heard of an agency not paying overtime, so there may be different laws for different states.
There are some agencies that do provide vacation/paid time off hours. Ratios of hours worked to vacation time vary-
This is always something to discuss with the agency during hiring process, or before.
