1. The physican orders Phenytoin (Dilantin ) 1 gm, PO,loading does (LD) in three divided doses over 24-hours period. The safe parameter for a LD of Dilantin is 15-18 mg/kg. The patient wieghs 132 lbs. Dilantin is available in 100 mg capsules.

A.How much does patient weigh in kgs?

B. How many mgs would the patient recieve in 24 hours?

C. Is the dose within safe parameters? Explain why?

2. The physician orders Cytoxan 200 mg, PO, bid. Safe administration is 1-5 mg/kg/day. The patient weighs 100 lbs and the drug is available in 50 mg tablets.

A. How much does patient weigh in kgs?

a. how much does patient weigh in kgs?

conversion factor:
1 kg = 2.2 pounds

132 pounds/1 x 1 kg/2.2 pounds
(conversion factor) =
60 kg

b. how many mgs would the patient receive in 24 hours?

conversion factor: 1 gram = 1000mg

so, the answer is
1,000 mg

c. is the dose within safe parameters? explain why?

to determine this you need to compute the low (15mg/kg) and high (18mg/kg) safe doses for a 132 pound (60kg) man and see if the ordered dose of 1,000mg falls within that range.

15mg/kg x 60kg = 900mg

18mg/kg x 60kg = 1080mg

safe range is 900 to 1080 mg per 24 hours. is 1000 mg within that range?
yes.

the physician orders cytoxan 200 mg, po, bid. safe administration is 1-5 mg/kg/day. the patient weighs 100 lbs and the drug is available in 50 mg tablets.

a. how much does patient weigh in kgs?

conversion factor:
1 kg = 2.2 pounds

100 pounds/1 x 1 kg/2.2 pounds
(conversion factor) = 45.4545, round off to
45.4kg

b. is the dose cytoxan within the safe parameters?

to determine this you need to compute the low (1mg/kg) and high (5mg/kg) safe doses for a 100 pound (45.4kg) man and see if the ordered dose of 200mg falls within that range.

1mg/kg x 45.4kg = 45.4mg

5mg/kg x 45.4kg = 227mg

safe range is 45.4 to 227 mg per 24 hours (a day). 200 mg bid = 400 mg/day. is 400 mg within that range?
no.

c. how many tablets would you give?

none. i would call the doctor and question this order.

thank you so so much for the help! i wish you lived in michigan i would pay you to help me. p.s the way that you write your problums makes it easy for me to understand it. anyone in the detroit mi area reading this that has passed the math exam let me know because i need a tutor.

This is pretty rudimentary stuff that any decent chemistry tutor could walk you through. Talk to the chem teacher at the local high school or junior college -- they might be interested in tutoring or they might be able to recommend one of their students. You might have to explain what "PO bid" means but other than that, they should be able to explain things to you.

I've also seen some online tutoring programs but I've never used one so I have no idea as to their quality.

Certainly poke around those threads to which Daytonite directed you... there's a ton of stuff to practice with.

By all means, though, start with your own instructor(s) and the tutoring center at your school.

I am so happy to know that there are people out in the world who trys to help others without gaining anything from it. Would you beleive that my school does not have tutors for this class?! I have had a teacher every now and than help me with a problum but that is it. I was told to buy a book and learn it on my own and I almost passed it because I had a 75% first time, 87% the second time. I will check with my schools chemistry department asap. Thank you

Mrs. Dawson Dr. ordered a stat order for Demerol 50 mg to be given IM with 25mg of Phenergan. The medications are compatable and both can be given in one injection. You have Demerol 100mg/ML vials and Phenergan 25mg/ML amples.

A. Which medication should you draw up first?

B. How many ML of each medication are calculated to be drawn into the syringe?

C. What is the total amount of fliud in your syringe when both are into the syringe?

Number 2

Mrs Jones Dr. has written an order for 0.2mg of Methergine IM to be given every 2-4 hrs. To a maximum of 5 doses. The pharmacy sends the 0.2mg/ML ampules to the floor.

A. How much will you draw up for the first dose?

B. Calculate the total amount of Methergine Mrs. Johns will recieve if she is given the entire five doses within the next 20

Sorry about that I meant to end the 2 nd question with 20 hours.

My suggestion would be to try to work out the problem to the best of your ability and then have someone check it. I'm sure that there are some of these questions you can answer. Daytonite and others are willing to help, but you can't expect them to do your homework totally.

If you don't get them right, no one is going to think less of you...med calculations take a lot of practice and then believe me, once you get it, you won't lose it (in my opinion).

a. which medication should you draw up first?

well, what do you think? will you be using the full container of either medicine or will you have some left? what issues might occur if some medicine were left in one container and you wanted to use it later?

b. how many ml of each medication are calculated to be drawn into the syringe?

if there are 100 mg of demerol per ml and you only want to draw 50 mg how many ml will you draw? 2 ml? 5 ml? 10 ml? maybe 0.1 ml? 1 ml has 100 mg, how many ml has only 50 mg?

if there are 25 mg of phernergan in 1 ml, how many ml do you need to draw in order to have 25 mg of phenergan?

c. what is the total amount of fliud in your syringe when both are into the syringe?

if you draw up x ml of demerol and y ml of phenergan, how many ml do you have in total? x + y, right?

let's see, she needs to get 0.2 mg and the pharmacy sent us ampules in which each ml contains 0.2 mg. so how many ml does she need in order to receive 0.2 mg?

b. calculate the total amount of methergine mrs. johns will recieve if she is given the entire five doses within the next 20 hours

so one dose is 0.2 mg... so how many mg are in two doses? three doses? five doses?

i think it was very sage advice that you try to work the problems. i've given you some things to think about that may lead you in the right direction.

i'll bet you can do these. try to reason out the answers before you start trying to set up the equations.

it really does make sense... i promise.

Lol, oh no this is not my homework it is some problums that I have ran across and attempted to do but am not sure that I did them right. I am most confused on the wording in each problum. I had 66 problums to try and I am coming across a problum or two that I am not feeling confident about. My school did not give us problums to do to get ready for the math exam that is coming up the end of Aug 2008. They expect us to do it on our own. I can`t even get any teachers to tutor me this summer.
