I am needing help with care plans for my last semester of LPN school. Any help will be greatly appreciated.
1st diagnosis is: altered parenting related to inadequate attachment
2nd- Potential for activity intolerance aeb hematocrit, hemoglobin levels (low)
3rd- Nutritional alteration less than body requirements aeb hematocrit, hemoglobin levels (low)
4th- family process alteration related to inclusion of new member

Is this based on a specific pt or just generalized? What exactly do you need help with? Need more info

I would begin by researching these in your OB text book or a Nursing Care Plan book. Good luck! These things ARE pretty basic.

Being that you are in school, and that you want to be a nurse....
We won't do your homework for you, but, I will give you some helpful Careplan Pointers...
A careplan basically summarizes four things:
1) What is going on, and Why is it going on (usually a specific situation)
2) What are you going to do (usually a list of things... most of them are common sense, and are ALWAYS related to what the MD/Policy says to do. {IE when the diaper's wet, what do you do.... Change it})
3) How can you tell what you are doing is working,
and finally
4) How long will it take for you to figure out if what you are doing is or is not working, and for how long is it necessary to see results...
So it usually ends up as:
Nursing Dx / What we're gonna do / What I expect to see / How long do I expect to see results in, and how long do I expect to see those results happen.
I hope this helps... (yes, its my pretty standard answer...)

I would love to help but I'm not sure what you are asking for. All you have listed are some nursing diagnosis there is no data.
Good data gathering is the key. If you have done a good assessment and read the chart your care plan becomes easy. The information you gather will point you to diagnosis.
Step 1: Read the chart, don't copy down a bunch of lab values look for problems that you can address in care plan. You have done this partially but did you make sure that her anemia is actually anemia? Or is it just the normal drop in H&H from pregnancy blood loss after delivery? If it is true anemia what kind is it? Not all are related to nutrition so. Altered nutrition might not be an appropriate diagnosis or concern for this patient. You can figure out what kind it is by looking up some of the other measurements of a CBC in your lab book.
Step 2: Interview the patient. Ask them what their concerns and needs are. I always ask my patient "Whets going on?" and when appropriate "Is there anything I can do for you" these help allot in identifying problems which can evolve into diagnosis. For a care plan on pregnant/post partum women you can find social and educational diagnosis very easily if you ask the right questions.
Step 3: do a physical assessment. This will change depending on what your patient is in the hospital/ clinic for.
After that its easy use your data to identify the top 3 or 4 problems correlate them with Nursing diagnosis then explain how you will solve them with nursing interventions and then how you will make sure that what you did worked.
I don't think the diagnosis you have listed will fly with your instructor. As I said before there is a normal drop in H&H for pregnant women so unless you back up the anemia one it wont fly. Also the activity tolerance is a bit of a stretch. Not only because it hinges on the questionable anemia but also because most young patients do fine unless their crit is really low.
As for the inadequate attachment one I cant say for sure because there is no data to support it but if this is post partum you had better make sure you have strong data. People bond with their children differently and what may seem to one person as altered is very normal for another.
Family process alteration related to inclusion of new member- this is a good one and you can load up the interventions.
As someone else said we wont do your homework for you but if you put more information in your post people would be able to help you more. GL on the care plan!

I also don't think the diagnoses you have listed are very appropriate. Did you come up with those on your own regarding a particular patient, or did your instructor give them to you based on a case study? If you came up with them on your own, I would definitely rethink at least #1, #2, and #3, unless of course you have objective data that leads you to believe the mom or dad is not bonding with the baby. We were taught to prioritize our diagnoses based on the immediate needs of the patient. And, you don't have to make each one specific to maternity. For example, if the mother has had an episiotomy or a c-section, she has impaired skin integrity and is at risk for infection. She is also going to be having quite a bit of pain. If she is on narcs, she might be at risk for constipation. These are just a few, there are many more that may apply to your patient. If you are really having trouble coming up with some, you can always go with that old standby,knowledge deficit, because there is sooooo much to teach to a new mom. Best of luck to you!
