For school we are making care plans for clinical, the teacher told us for the long term goal to make it up. So if my long term goal is for 3 months, what do I put in the evaluation as the long term goal...LTG still ongoing? or do I put nothing since I am still "evaluating" it? Thanks so much!

I have no idea what your instructor wishes. I would think...process ongoing or currently evaluating in process would suffice. This may be better asked of him/her.

I'd ask your instructor, but let's think of a familiar example.
You have a three-year-old. You are in the process of potty training her. What are your short term goals? Things like, oh, being able to recognize when she's wet.
Then to recognize that feeling that she feels just before she feels wet.
Then to understand that means she needs to go to the potty.
Then to tell you when she needs help to go to and use the potty.
Then to be able to doff and don underwear herself.
Then to wash her little hands before she leaves the bathroom.
Long term goal: TO BE INDEPENDENT in toileting with no more than one accident per week. You estimate this will take six months. So your evaluation criterion is: She's independent with no more than one accident per week achieved on DATE.
