I was taught in school LVN/LPN's were not allowed to assess. I keep hearing LVN's at work trying to make the case that they can. What are the limits to the tasks the LVN can be delegated?
if the patient is comming from our ER then they were initally assessed by an RN (no LPN's in the ER) so an LPN can take the admission and everything is signed off by an RN and an RN has to take the LPN's patient assigment at shift change, if the pt is a direct admit from a doctor's office then they give the new admit to an RN, if the RN's case load is full then the LPN will take it but the shift supervisor OR the RN who is covering the LPN will do the admission assessment.

Agree with Nancy Nurse. It is the same at my facility.
When I get into the LVN-RN bridge I can't wait to see how the assessment changes. :)
